Swab
Collection Kit
When you place an order online or by phone using a
credit or debit card, we send you our swab collection
kit. We do not send out free kits but invite you to make
your own kit if you are in a hurry and don't want to
wait for a our kit.
Instructions are provided in your collection kit with
some information required to be filled in. Below are the
details of the instruction page.

The collection kit contains the following (2

2 sets of sterile cotton-tipped swabs
|
| |
2 sample envelope |
| |
1 pre-paid/addressed return envelope |
| |
|

Open 1 set of sterile swabs. Rub the 2 swabs in
the inner |

cheek with an up-and-down motion for 20 seconds.
Place |
| |
the swabs in a sample envelope and seal with
tape. |
| |
|

3. |
Complete the information on the envelope. It is
important |
| |
that the information be complete and the
envelope is |
| |
signed and dated. |
| |

4. |
Use the remaining set of swabs to collect from
the next |
| |
person following steps 2 and 3. |
| |
|
|
5. |
Place the completed sample envelopes and the
contact |
| |
information into the return envelope. Seal and
send. |

your results once the final report has been written. You
will be asked to verify your password for any phone
conversations. We can not
