STEP

INSTRUCTIONS

 

 

1.

You will need the following:

A. Two (2) cotton-tipped swabs, such as Q-tips, for each person.

B. Two (2) envelopes. One for each person.

C. One (1) envelope to send samples to the lab.

 

 

2.

Rub the inside of your cheek for 15 seconds with both of the swabs. Place wet swabs in one of the envelopes. DO NOT PLACE SWABS IN PLASTIC.

 

 

3.

Seal the envelope and label it with the date, name, race, and relationship to the case (alleged father, child, sibling1, etc.). Complete the "Send with Samples" form and attach to envelope. It is important that this information be complete and that the form be signed and dated.

 

 

4.

Repeat steps 2 and 3 to collect DNA from the second person to be tested.

 

 

5.

Place both completed envelopes into the return envelope along with the appropriate payment (see Prices). Seal the envelope and mail to:
